Signature Initial The Waggin Train Veterinary Clinic - Terms & Conditions The Waggin Train Veterinary Clinic Client Consent & Agreement Form 1. Cancellation Policy Please notify us at least 24 hours in advance to cancel or reschedule. Veterinary Appointments: $50 late fee. Anesthetic Procedures: $120 late fee. Grooming Appointments: $35 late fee (or 1/2 base price). I understand the cancellation fees and notice requirements. 2. Vaccine Requirements (Elective/Drop-Off) Records must be verified 48 hours prior to appointment. Annual Rabies (Feline/Canine) Annual DHLPP (Canine) Bi-Annual Bordetella (Canine) Annual FVRCP (Feline) Note: If not up to date, vaccines must be administered at least 2 weeks prior to surgery. Upload Vaccination Records (Optional): I agree to provide proof of vaccinations as required. 3. Pet Grooming Policy Sedation & Special Handling Cats are automatically sedated for safety. Anxious/aggressive dogs may be sedated if necessary. Special handling fees apply for uncooperative pets. Health & Safety (Fleas/Matting) Pets with fleas will be treated with Capstar at owner's expense. Matted pets may be shaved for their safety; owners acknowledge risks of hematomas or skin irritation. Consent to Treat I give full consent to treat in case of grooming injury or medical emergency. I accept the grooming terms, including sedation and emergency treatment consent. 4. Media Release Form I consent to The Waggin Train capturing and using photos/videos of my pet for promotional or social media purposes without compensation. I agree to the Media Release (Optional).
